Medical Supplement Plan
This is an insured benefit from American Public Life Insurance Company.
For Members enrolling in the current month, the benefit becomes effective on the first day of the next month.
Outpatient Physician Benefit
This pays $25.00 for treatment of covered sickness by a Physician in the Physician's Office, Clinic, Urgent Care Facility or Emergency Room, subject to a 5 visit maximum per person and a maximum of 10 visits per family per calendar year.
Daily Hospital Benefit:
This pays a daily benefit of $30.00 or $900.00 monthly for hospital confinement as an inpatient due to a covered accident or sickness up to a maximum of 180 days per confinement.
